This form must be completed in full to submit to Teleradiology Q/A

1. Please be sure that the patient identifying INFO is accurate or the case may be unable to be retrieved.
2. Once submitted and reviewed, a member of the Q/A committee will give F/U to BOTH radiologists
3. DO NOT enter case in RADPEER database (this will be done by QA Committee)
